I forgot to cut the bread top prior to putting it in the oven.
To exacerbate the issue, I put the bread in the oven and then turned on the heat. As a result, the crust dried before
I set out the starter for a day at room temperature. I think this helped the bread consistency. Everything after that was a sequence of mistakes.
Ingredients:
1 cup of unbleached flour, 2 cups of bread flour (I unintentionally used non-bread flour)
1 teaspoon of salt
2.5 teaspoons yeast
cold water which had sat in the fridge overnight

1/2 cup sour dough starter
water
mix with bread mixer
in fridge 12 hours, loosely covered with Saran wrap
remove from fridge
no kneading, score with knife, let rise in glass pan for 2 hours uncovered
place pan in oven at room temperature
heat oven to 455F, bake until golden brown
description of result:
bread crust is dry and unappealing
texture of bread is dense; few air holes

remove from fridge; knead by hand, let rise in glass pan for 2 hours uncovered
place pan in oven at room temperature
heat oven to 455F, bake until golden brown
description of result:
bread crust is smooth and even
texture of bread is airy and flaky; small to medium air holes
observation: kneading manually after removing from fridge yielded good density and crust
